Parents are welcome to print and complete their child's registration forms prior to their visit to our office. Please remember to bring the completed forms along with your child's dental insurance information to your child's appointment. If you are unable to complete the forms prior to the visit, please plan to arrive 10-15 minutes before your child's appointment time so that you can complete the forms in our office.
